U.S. Students – 1098T Forms
1098Ts for the 2012 tax year were issued by ECSI. Students who elected to have their 1098T sent electronically should receive an email prior to January 31, 2013. For students who did not elect to receive their 1098T electronically, your form was mailed on or before January 31, 2013 to the address that we have on file.
If you did not receive your 1098T or if you need additional copies, please log onto ECSI. To access your 1098T you will need the school code for D'Youville which is VH, your account number which is your social security number (SSN) and PIN/Password. If you do not have your PIN/Password, retrieve your login credentials here.

Canadian Students – TL11C Forms
TL11Cs for the 2012 tax year are issued by D'Youville College. TL11C forms are available for pick up in the Student Accounts Office during normal business hours until January 30, 2013. Any form not picked up by January 30, 2013 will be mailed to the address on file.

Students Who Worked on Campus – W2 Forms
W2 forms for students who worked on campus are available for pick up in the Student Accounts Office during normal business hours until January 30, 2013. After January 30, 2013, W2 forms will be mailed to the address on file. If you have recently moved, please notify Human Resources immediately.
