Application Instructions

  1. Complete the Online Application
    Please complete the online application. The $25 application fee is waived for online applicants.
  2. Submit Additional Materials
    Freshman Applicants:
    » Forward one official copy of your high school transcript
    » Forward SAT and/or ACT scores. (Contact your guidance office for assistance.)
    » Letters of recommendation are welcome but optional.
    College Transfer Applicants:
    Submit one official copy of each transcript from all colleges attended.
  3. Health Requirements Compliance
    New York State requires proof of immunization for measles, mumps and rubella for everyone born after 1956. Please have your physician complete the health form and return it to the Health Center as soon as possible. A completed health form must be submitted prior to registration. For more information, please contact the Health Center.
  4. Physician Assistant Applicants
    Applicants to the Physician Assistant program are required to submit a departmental application in addition to the online application requirements.

All documents should be mailed to the Undergraduate Admissions Office.
